Skilled Nursing Services in Your Own Secure Setting
If you need home health care from a registered nurse for general medical or post-surgical conditions, Visiting Nurse Association (VNA) is pleased to bring our skilled nursing services to you.
In addition to direct patient care—which includes specialty wound care, medication management, IV therapy and more—our teams of general medical and surgical nurses provide the following skilled nursing services within your home:
- Initial in-home assessments to determine services needed and develop a plan of care (upon request)
- Ongoing communication and coordination with primary care providers
- Regular reevaluation of patient needs and adjustments to the plan of care as needed
- Educational guidance for the patient and family or regular caregiver
