Visiting Nurse Association strives to make our home health referral process as simple as possible.
Referrals can be made online by submitting the appropriate form on the left side bar. Additionally, referrals can be made by calling our VNA Customer Referral Center (CRC) at 816-627-6210 or fax the CRC department at 816-627-6235.
If you are a physician's office, please email us to request a fax referral pad be sent to your location.
