To receive VNA home health care services, every patient must meet general admission guidelines.

General VNA Admission Guidelines

  • Have services ordered by a physician.
  • Require skilled nursing or rehabilitation therapy that is medically necessary.
  • Need care provided on an intermittent (not daily) basis.
  • Be homebound (read below to learn more about homebound status definition).
What is homebound?
Homebound is defined as:
  • A patient not normally being able to leave home;
  • If a patient does leave home, it is an extremely taxing effort; and
  • Needs the assistance of another person.

If a patient must leave home it must be:

  • Infrequent;
  • For a short period of time; or
  • To receive medical care

Insurance companies can vary on admission guidelines. Patients need to be aware of what their insurance policy covers and requires in order to receive home health care services.

Skilled Nursing

General Medical/Surgical Nursing
Our teams of general medical and surgical nurses provide direct care, teaching and assessments to our patients.
Specialty Nursing Programs
Enterostomal Therapy Nursing- specialty trained, registered nurses in wound care, ostomy care and continued treatment.
Intravenous Therapy Nursing-specialty trained, registered nurses in infusion drug administration and line maintenance.  VNA Plus is our home infusion pharmacy. VNA Plus is a wholly owned subsidiary; for more information go to www.vnaplus.com.
Mental Health Nursing- specialty trained, registered nurses provide in-home mental health care for patients who are experiencing acute episodes or exacerbation of mental illnesses.
 

Rehabilitation Services Program

VNA offers a comprehensive rehabilitation program including physical, occupational, and speech therapy. Additionally, VNA offers a Total Joint Replacement Program to any patient preparing for hip or knee replacement surgery.
  • Physical Therapy: The use of exercises and physical activities to help condition muscles and restore strength and movement. For example, physical therapy can be used to restore hip and knee movement and build back strength after replacement surgery.
  • Occupational Therapy: Helps patients with a permanent or temporary impairment in physical or mental functioning to perform daily tasks in their living environments (i.e. bathing, dressing, grooming, etc.). Occupational therapy assists in the development of skills needed to live independent, satisfying and productive lives.
  • Speech Therapy: Treatment of speech defects and disorders, especially through use of exercises and audio-visual aids that develop new speech habits.

Medical Social Work Program

Master's prepared social workers evaluate and assist the patient and their family in the social, economic and emotional needs relating to the overall medical plan.

Nutritional Program

VNA registered dietitian provides nutritional assessment and diet treatment.

Home Health Aide

Home health aide services, such as bathing, grooming, light meal preparation, changing bed linens, and light laundry are provided to patients.

Total Joint Replacement Program

  • A home visit before surgery by a physical and occupational therapist to assess your physical condition and home environment, as approved by payer;
  • A therapist provides educational material on the care you will need to recovery from surgery and home safety tips;
  • An individualized treatment plan created for you by your physical and occupational therapist;
  • After hospital discharge, therapists will provide physician ordered therapy and education on activities of daily living.

TeleHealth Home Monitoring Program

The easy-to-use monitor takes your vital signs (blood pressure, heart rate, oxygen saturation, temperature and weight) every morning. Next, a friendly voice asks up to ten “yes” or “no” questions, individualized to the patient’s condition, which allows VNA to further evaluate their health. Results are then transmitted electronically via phone line to a nurse at the VNA Central Station.
