VNA measures the health outcomes of our patients each quarter. Those outcomes are utilized to determine the strengths and weaknesses of services rendered during that period of time. After reviewing outcome results, strategies are implemented to strengthen programs and overall quality of care in every aspect of services provided.
Below are outcome results for VNA today, last year and the national averages.
VNA is proud to announce:
- VNA patients are both admitted to the hospital and use the emergency room less than the national reference.
- Only, 26.4% of VNA patients had to be admitted to the hospital compared to the national reference of 31.8%.
- VNA discharged our patients back into the community more often than the national average. 70.7% of VNA patients are discharged to the community compared to a national reference of 65.1%.
VNA will continue to pursue an even greater reduction in the use of emergent care and re-hospitalization in 2006.
Patient Satisfaction
As apart of our mission to provide your patients with the highest quality of care, we measure patient satisfaction throughout the year. The results are reviewed quarterly and compared to previous months for constant improvement. Below is how we scored last year.
