VNA TeleHealth Home Monitoring The VNA TeleHealth Home Monitor takes a patient’s vital signs such as: blood pressure, heart rate, weight, temperature and oxygen saturation on a daily basis. The monitor sends the results to the VNA Central Station electronically via phone line. The results are reviewed daily by a nurse who responds to programmed “alerts” by calling the patient and providing consultation.

Electronic medical records VNA uses electronic records to access shared information through laptops given to each of our visiting clinicians. Patient information is updated at each home visit to keep staff informed of a patient’s status. Additionally, for patients seeing multiple clinicians, electronic medical records avoid scheduling conflicts.
Wound VAC Our Wound and Ostomy care team consists of nurses who specialize in the care of patients with burns, complex wounds and ostomies. Services available include caring for wound care patients utilizing vacuum assisted wound closure (VAC), teaching patient’s and their families how to care for ostomies and recommending treatment plans for decubitus ulcers.
Digital Photography VNA uses digital photos in cases such as wound care to better explain a patient’s progress to the physician. Digital photos are taken and sent to the physician via email to discuss recovery and plan of care. |
