VNA Services

Services provided by Visiting Nurse Association

 
To receive VNA home health care services, every patient must meet general admission guidelines.

General VNA Admission Guidelines

  • Have services ordered by a physician.
  • Require skilled nursing or rehabilitation therapy that is medically necessary.
  • Need care provided on an intermittent (not daily) basis.
  • Be homebound (read below to learn more about homebound status definition).
What is homebound?
Homebound is defined as:
  • A patient not normally being able to leave home;
  • If a patient does leave home, it is an extremely taxing effort; and
  • Needs the assistance of another person.

If a patient must leave home it must be:

  • Infrequent;
  • For a short period of time; or
  • To receive medical care

Insurance companies can vary on admission guidelines. Patients need to be aware of what their insurance policy covers and requires in order to receive home health care services.

Skilled Nursing

General Medical/Surgical Nursing
Our teams of general medical and surgical nurses provide direct care, teaching and assessments to our patients.
Specialty Nursing Programs
Enterostomal Therapy Nursing- specialty trained, registered nurses in wound care, ostomy care and continued treatment.
Intravenous Therapy Nursing-specialty trained, registered nurses in infusion drug administration and line maintenance.  VNA Plus is our home infusion pharmacy. VNA Plus is a wholly owned subsidiary; for more information go to www.vnaplus.com.
Mental Health Nursing- specialty trained, registered nurses provide in-home mental health care for patients who are experiencing acute episodes or exacerbation of mental illnesses.
 

Rehabilitation Services Program

VNA offers a comprehensive rehabilitation program including physical, occupational, and speech therapy. Additionally, VNA offers a Total Joint Replacement Program to any patient preparing for hip or knee replacement surgery.

Medical Social Work Program

Master's prepared social workers evaluate and assist the patient and their family in the social, economic and emotional needs relating to the overall medical plan.

Nutritional Program

VNA registered dietitian provides nutritional assessment and diet treatment.

Home Health Aide

Home health aide services, such as bathing, grooming, light meal preparation, changing bed linens, and light laundry are provided to patients.

Total Joint Replacement Program

The VNA Total Joint Replacement Program provides maximum assistance to physicians and their staff in helping patients deal with their preparation for orthopedic surgery and post operative care.
  • VNA provides physician office support through 24 hours a day, 7 days a week availability for patient inquires/concerns;
  • Thorough patient/family education increases patient compliance;
  • Home visits enable therapists to see first-hand daily challenges for the patient, enabling VNA to create an individualized treatment plan including DME;
  • Regular communication between the patients’ physician and VNA team to provide an effective plan of care for the patient.

TeleHealth Home Monitoring Program

TeleHealth Home Monitoring allows VNA to monitor a patient’s vital signs: weight, blood pressure, heart rate, oxygen saturation and temperature, on a daily basis. Results are sent to the VNA Central Station via phone line then tracked, recorded and faxed to the physician for review.
