What we do
The Visiting Nurse Association provides medical support and education to those returning home from the hospital or living with a chronic illness. VNA services are provided on an intermittent basis and require a physician’s referral.

Services available through VNA may include, but are not limited to, the following:
Home Health Candidates, not limited to the following:

Advanced age
-
Lives alone or with elderly spouse

Repeated hospitalizations
-
Prolonged hospital stay
-
New diagnosis or surgery
-
CHF or COPD diagnosis
-
Suffering from a chronic illness
-
Diabetes: unstable or newly diagnosed

Fractures requiring therapy
-
Total hip or total knee replacements
Some of general requests for home health care services have included:

Rehabilitative Therapy
-
Wound care, dressing change
-
Instruct and assess reaction to change in medication

Cardiopulmonary assessment
-
Routine catheter care
-
Ostomy education and care
-
Caregiver training and home safety consultation
-
Nutritional Counseling
-
Lab work secondary to a skilled nursing visit or in response to unstable condition

Educate patient and family about signs and symptoms of exacerbation of disease
